2011- 12 Academy Registration Downloads

To register a US Soccer Development Academy player for 2011-12, complete and mail to:
Michelle Klein, 6058 Bayberry Ave, Manheim, PA 17545

To register a player for the U16 or U18 Academy teams, I will need by August 15th

- color photo

- copy of birth certificate or passport

- US Soccer Waiver form. Please make sure it is signed and dated by both the player and parent.

- copy, front and back, of health insurance card.

- email address, nationality and citizenship written on the back of the US Soccer Waiver

- notarized PA Classics Medical Release

If a player was born or played in a club outside of the country, please contact the club registrar, Michelle Klein, at klein17036@yahoo.com. He may need International Clearence before being issued a player pass.
