To register a US Soccer Development Academy player for 2009-2010, complete and mail to:

1. color photo with last name on back
2. 2010-11 US Soccer Waiver form 
3. PA Classics 2010-11 Medical Release Form (MUST BE NOTARIZED)
4. copy, front and back, of health insurance card
5. copy of birth certificate or passport

If a player was born or lived outside of the 50 states:

Please email our club registrar, Michelle Klein, at klein17036@yahoo.com prior to registration night if a player WAS BORN OVERSEAS. US Soccer will need to clear the player before he will be issued a Player Pass. The clearance process can take up to 30 days, so let Michelle know ASAP so she can let you know what additional forms you will need to fill out and bring with you to registration.
